To preregister for OSF St. Joseph
Medical Center Birthing Center, please follow these steps:
1. Print the Adobe Acrobat file
(click
here), which contains the Birthing
Center Pre-Admission Questionnaire. Fax the form to 309-662-5821 or
mail it to:

2. Stop by the Admitting & Registration Department at OSF St. Joseph
Medical Center to sign the necessary Consent for Treatment forms in
advance. 3. Call Sarah Musselman at 309-665-4704 to insure that your questions
are answered and all necessary information has been obtained.
4. When you come to the hospital to have your baby, enter through the
Emergency Department. They will escort you to the Birthing Center.
