Diabetes Self Management Education
Program
The program is coordinated by our Diabetes Educator, a Registered Nurse
who is board certified in Advanced Diabetes Management. The program
includes individual consultation with the Diabetes Educator, as well as
group sessions. Topics include understanding and living with diabetes;
risks of poor glucose control; skin/foot care; nutrition; medications;
relationships between activity, nutrition and medications; and goal

Eat Right For Life Nutrition
Counseling
OSF St. Joseph Medical Center offers one-on-one counseling with
registered and licensed dietitians who specialize in personalizing a
nutritional care plan for you. This nutrition counseling can focus on
disease specific issues if indicated. In most cases a physician referral
is required. To request a referral, ask your physicians to contact the
OSF Dietitian Scheduling Office at 309-662-3311, Ext. 4046. OSF staff
will then contact you to schedule a convenient personal appointment time
with Registered Dietician Kim McClintic
or a member of her staff.
Stress Management Counseling
Learn skills to help handle stress and live in a way that will enhance

Exercise Counseling
These sessions are performed by an exercise physiologist. OSF St. Joseph
staff work with participants to monitor the amount of exercise in their
day-to-day activities and establish new health goals. Utilizes the
10,000 Steps Program. For more information about using exercise to build

Smoking Cessation Counseling
The Center for Healthy Lifestyles offers a seven-week smoking cessation
program call “Freedom from Smoking.” Developed by the American Lung
Association and led by a trained professional, this program offers
support for those who are kicking the habit and dealing with the
emotional, psychological and physical effects of giving up cigarettes.
